The May 2026 issue of Pediatric Critical Care Medicine (PCCM) features four original research articles from a variety of topics, including earlier pediatric intensive care unit (PICU) exposure in residency, adherence to sedation protocols, standardizing neonatal ICU-to-PICU transfers, and characterizing quality of bag-mask ventilation before intubation. There are six clinical investigation studies that cover the following topics: 1) development of a large language model to estimate functional status scale scores, 2) characteristics of prolonged noninvasive respiratory support after the Norwood procedure and clinical outcomes, 3) bolus epinephrine dose-readiness for cardiac prevention in cardiac ICUs, 4) prevalence of terminal CPR in the final hours of life and the decision to forego it, 5) association of albumin infusion and clinical outcomes in patients with and without septic shock, and 6) identification of inflammatory proteins in severe hypoxemic respiratory failure and their influences on pulmonary endothelial integrity. The issue concludes with two PCCM trials: one trial measures PICU strain and associations with patient outcomes, and the other trial measures clinical and cost outcomes associated with withholding routine measurements of gastric residual volumes while feeding intubated patients.
