Severe respiratory failure is a well-described complication in the trauma population and presents unique challenges that require careful consideration when implementing conventional ARDS therapies. Management must account for individualized carbon dioxide targets when lung-protective ventilation conflicts with cerebral perfusion goals, particularly in patients with traumatic brain injury. When neuromuscular blockade is required, invasive intracranial pressure monitoring may warrant consideration in patients at risk for intracranial hypertension. Finally, in patients with refractory respiratory failure, venovenous extracorporeal membrane oxygenation (VV-ECMO) can serve as an effective rescue strategy and may be successfully employed using anticoagulation-free approaches when bleeding risk is prohibitive.

Disagreements, differing perspectives, and interpersonal conflict are common in the intensive care unit, particularly when patients are critically ill, prognostic uncertainty is high, or treatment decisions carry significant ethical implications. In these situations, achieving alignment among clinicians and fostering constructive dialogue can be challenging. Differences in communication styles, professional roles, and emotional responses may further complicate team interactions and decision-making. Despite the frequency of these challenges, the critical care literature offers limited guidance on navigating difficult professional interactions, managing conflict across disciplines, and responding effectively to strong emotions in clinical discussions. This webcast will explore practical approaches to fostering respectful communication, addressing conflict constructively, and maintaining professionalism in emotionally charged ICU environments. Through case-based discussions submitted by SCCM members and a review of the relevant literature, participants will gain tools to strengthen interprofessional relationships and support effective teamwork in critical care settings. This webcast is hosted by the Ethics Section.
Non–SCCM Event Sepsis
The Oregon SCCM Chapter is hosting their annual symposium, which includes a virtual component. The chapter is partnering with Sepsis Forum- Bench, Bedside, Beyond to sponsor the conference. IFAD 2026 will mark the 15th anniversary edition of the meeting and introduces a renewed educational concept focused on interactive learning, hands-on education, physiology-based discussions, and multidisciplinary fluid stewardship. The meeting aims to bring together intensivists, anesthesiologists, emergency physicians, surgeons, nurses, pharmacists, and allied healthcare professionals with a shared interest in hemodynamic management and fluid therapy.
This advanced session explores the recognition and management of high-stakes hematologic complications in critically ill patients. Moving beyond the basics, we will focus on complex diagnostic and therapeutic challenges at the intersection of hematology and critical care.
