Hosted training opportunities are delivered by institutions through the work of qualified instructors, directors, and consultants. Each of these educational roles has specific responsibilities and eligibility requirements, which are outlined below.

Note: Instructor, director, and consultant status are maintained by maintaining eligibility requirements and teaching in or directing at least one hosted course every two years.

Note: Once an individual is an-SCCM approved instructor for one Fundamentals course, he/she can become an instructor for another Fundamentals course by completing that course and submitting an instructor application for that course.

**Clinical practice and labor and delivery management refers to the management of pregnant patients, including inpatient management of labor, performance of deliveries, and care of the postpartum patient. Critical care-certified practitioners will be accredited as instructors on successful completion of all criteria listed in this document. Instructor applicants with less than 50% critical care practice ratio requirement may require a review before instructor status is granted.

All Hosted Training: Confirmation of critical care practice ratio form
This form is needed with the instructor application if an instructor candidate needs to show a minimum 50% care practice ratio. It should be completed by the candidate’s ICU director, department chair, division director, chief medical officer, or hospital administrator.
FCCS: Obstetrics Only: Confirmation of labor and delivery practice ratio form
This form is needed with the FCCS: OB instructor application if an instructor candidate needs to show at least 50% clinical practice and labor and delivery management. It should be completed by the candidate’s department chair, division director, chief medical officer, or hospital administrator.
FCCS: Resource Limited Only: Confirmation of resuscitative medicine experience form
This form is needed with the FCCS: RL instructor application if an instructor candidate is a prehospital medic. It should be completed by an FCCS: RL director or consultant.
Ultrasound Only: Course recommendation form
Personal recommendation from a Critical Care Ultrasound director, consultant, or current or past course faculty member.
All Hosted Training: Course director recommendation form
This form is required when submitting a director application and should be completed and signed by an active course consultant.
FCCS: Resource Limited Only: Prehospital medics are not eligible to become FCCS: Resource Limited directors.
